Dental insurance is a benefit provided either through your employer or through an individual plan that you purchase. As your dental provider, we make every effort to verify your eligibility and benefits before providing treatment. While we are knowledgeable about many dental insurance plans, we are not experts in every policy and we do not have control over the specific details, limitations, exclusions, or clauses within your plan. Therefore, we cannot be held responsible for how your insurance company processes or denies claims, or for any unpaid balances resulting from your insurance carrier's policies.

The financial breakdown provided by our office is only an estimate of insurance benefits. Actual coverage may differ due to incomplete information from your insurance company or limitations within your policy. The patient is ultimately responsible for any balance remaining after insurance claims are processed. Insurance companies may deny claims based on plan provisions, policy guidelines, or medical necessity. Estimated insurance coverage and patient responsibility discussed by our office are based on information provided by your insurance company and are not a guarantee of payment. Final payment decisions are made solely by your insurance company once claims are received and processed, based on your eligibility at the time of service.

Any treatment recommended by your dentist is based on what he or she determines to be best for your oral health, not necessarily on what your dental insurance covers. Your insurance plan may or may not cover all aspects of the recommended treatment. The patient is ultimately financially responsible for any treatment not covered by their insurance plan.

Annual Maximum

Most dental insurance plans have an annual maximum benefit (typically 1000-2000 per year). Once you reach this limit, the insurance stops paying. We help you plan treatment to maximize these benefits.

Deductibles

You may need to pay a deductible (commonly 50-100) before your insurance kicks in. This applies per calendar year. Preventive care is often exempt from deductibles.

Coverage Tiers

Different services are covered at different percentages. Preventive care (cleanings, exams): 100 percent covered. Basic services (fillings, extractions): typically 80 percent covered. Major services (crowns, implants): typically 50 percent covered.
